Student Grievance Form
This form is for students to document and report incidents that violate our school policies or otherwise endanger the safety or well-being of our students and staff.
Students must report incidents immediately to a SIMA staff person
or
complete this form within 24 hours of the occurrence.
We review incident reports within 1-2 business days.
Students may report incidents anonymously.
If this is an urgent situation, please contact Guro Bob immediately or notify Anthony or Makis.
